Testicular cancer is the most common solid malignancy affecting males between the ages of 15 and 35, although it accounts for only 1 percent of all cancers in men. Testicular carcinoma is the most common malignancy in young adult men (aged 15–40 years). The natural course of testicular carcinoma is metastasis that eventually results in death. Most patients with testicular germ cell tumor present with a painless scrotal mass. 41-years-old man presented with swelling left side testis and tenderness left side groin. Laboratory investigations revealed a high normal red blood cell count 5.7x1012/L, CRP-10.5 mg/L. Hemoglobin, renal and liver function tests were normal. Ultrasounds showed 10.5x7.2 cm measured inhomogeneous mass without infiltration in surrounding fatty tissue in left groin. On pelvic and abdominal CECT showed 18.0x8.5x8.0 cm sized big mass through left testis and groin region, and showed enhanced by contrast paraaortal lymph nodes metastasis and 1.5x2.2 cm sized diameter metastasis in lung. A diagnosis of testicular germ cell tumor with multiorgan metastasis (lymph node and lung) was made. The natural course of testicular carcinoma is metastasis that eventually results in death.
