Introduction: Microalbuminuria has been highlighted for its clinical implication in the long-term prognosis of diabetic patients. Accordingly, microalbuminuria is routinely checked in diabetic patients in order to evaluate renal function and predict long-term prognosis. Microalbuminuria in patients with metabolic syndrome resulted in a higher prevalence of subclinical atherosclerosis and CV events8. On the other hand, the role of hyperinsulinemia, a possible risk factor of microalbuminuria, in the development of microalbuminuria in the general population is still debatable. To the best of our knowledge, there have been only a few cohort studies with large populations examining the relationship between fasting insulin resistance and microalbuminuria. Purpose: To investigate the association between insulin resistance and the development of microalbuminuria Materials and methods: This a prospective cohort study was done among diabetic adult patients who are under observation of Second State Hospital, Ulaanbaatar. Mongolia. A total of 120 patients (women 48.3 %, men 51.7 %) aged 64,9±8,6 were enrolled who visited during May to August 2019. Main criteria for study enrollment was a diabetic patient who injects insulin regularly under endocrinologist’s close observation. In terms of lab testing, fasting blood sugar and fasting insulin, glycated hemoglobin and microalbumin were used. Insulin resistance was calculated by Homa-IR (homeostasis model assessment of insulin resistance) (HOMA-IR = {insulin [mU/L] × fasting glucose [mmol/L]}/22.5) and it is considered as insulin resistance if Homa-IR was above 2.7. Microalbuminuria was (detected-41.6%) (Not detected – 58.3%). 20 of patients were diagnosed with chronic kidney disease 13% of whom were diagnosed with insulin resistance. Results: Pearson Correlation Analysis showed that insulin resistance correlated closely with microalbuminuria r(120)=0,1, p<0.05. Conclusion: Insulin resistance affects microalbuminuria according to the study of these authors Sung Keun Park. There was a study on insulin resistance and I, II phase insulin secretion but no study of the relationship between insulin resistance and microalbuminuria was found. The study showed the clinical utility of the insulin resistance as an independent predictor of microalbuminuria, which suggests the necessity for careful surveillance for microalbuminuria even in diabetic patients with hyperinsulinemia.
