Abstract

Background: As of 2021, 1 in 10 people worldwide, or 537 million adults (ages 20-79), are living with diabetes, and this number is expected to increase to 643 million by 2030 and 783 million by 2045. It is estimated that 6.7 million people will die from the disease in 2021, or 1 person dies every 5 seconds. In our country, as of 2021, a total of 800,000 people have participated in the early detection of type 2 diabetes, which is 76% of the eligible population. In our country, there is limited research on the health costs of type 2 diabetes, including the treatment and other health costs of retinopathy, so it is necessary to conduct this research. Aim: Calculation of treatment costs for type 2 diabetes Materials and Methods: We used a combination of top-down cost allocation and bottom-up cost allocation for health care costing in this study. In the study, one organization from each of the district, province, and regional diagnostic and treatment centers and referral level hospitals was included as a representative, and it was completed using the 2021 financial year report and the 2022 half-year performance financial report using a snapshot research model. Results: According to the results of the study, the cost of inpatient treatment for patients with 2 causes of CKD is calculated by one bed-day and one-time admission. The cost of inpatient care at the Central Hospital is 2,845,904 MNT, 787,608 MNT at the National Medical Center, 634,333 MNT at the General Hospital, and 643,213 MNT at the District Health Center. However, the cost of one bed per day is 353,090 MNT in the central hospital, 88,232 MNT in the district hospital, 85,239 MNT in the hospital, and 69,860 MNT in the general hospital. The average bed-days in the central hospital is 8.06, the average bed-days in the National Hospital is 9.24, the average bed-days in the general hospital is 9.08, and the average bed-days in the district hospital is 7.29. Conclusion: When calculating the cost of departments using the top-down costing method, the intensive care unit has the highest cost, and the average cost of all departments is 272,583 MNT. Direct labor costs account for 18 percent of the average cost, and drug costs account for 11 percent.
