Background: The smile also influences a person’s perceived attractiveness and is the cornerstone of social interaction. In traditional orthodontic treatment, orthodontists focus on teeth alignment, occlusal relationship, and function. Patients, however, are concerned about their facial esthetics, particularly smile esthetics. Most patients seek orthodontic treatment for esthetic purpose. Consequently, it is crucial for an orthodontist to be able to assess smile esthetics in a patient. So far, there have been no studies in the field of orthodontics that investigated the effect of smile esthetics on the different kinds of malocclusion for clinical reference. Therefore, in this study, our aim was to quantitatively assess the relationship of smile esthetic variables with angle class 3. Objective: To study some characteristics of the smile of people with Angle’s class 3 malocclusion. Materials and Methods: A total of 54 patients who fulfilled the inclusion criteria were included in this study. The participants were divided into the following 3 groups according to their age. Group 1 (20-24), group 2 (25-29), group 3 (30-34). Results: The smile arcs of all respondents consonant to groups 1, 2, and 3. The type of the smile is high in group 1, medium in group 2, and medium in group 3. The symmetry of the smile is symmetry in groups 1, 2 and 3. The buccal corridor is visible in group 1, but not in groups 2 and 3. The display at rest is defined as not visible in groups 1, 2, and 3. Conclusions: Most of the smile variables differed significantly among different types of age group. Angle class 3 malocclusion shows greater appearance of the buccal corridor when smiling due to advanced mandibular molars and maxillary retrusion.
