Abstract

Background: This study assessed whether the Noyes histological criteria dating method allows precise endometrial dating and optimal timing for embryo transfer. Materials and Methods: Histomorphological dating method was used for endometrial dating, with parameters including menstrual cycle days, and serum hormone analyses. Endometrial maturation was analyzed on day +5 after ovulation biopsies from 48 repeated implantation failure patients during the secretory phase and 52 fertile patients also serum hormone analyses of luteinizing hormone, follicle-stimulating hormone, estrogen, and progesterone. Results: The average age of women with repeated implantation failure was 38.75±7.1 years and 35.58±4.9 years in the health group out of 100 women surveyed. Serum hormone levels were RIF women E2 38.54±2.18 pg/ml, FSH 15.92±1.69 mIU/ml, OV day E2 289.43±28.1 pg/ml, LH 27.03±2.77 mIU/ml, P4 0.80±0.12 ng/ml, WOI day E2 143.91±10.21 pg/ml, P4 22.74±1.25 ng/ml, endometrial thickness 9.61±0.33 mm, menstrual cycle 29.10±0.12 days. Relatively healthy women were menstruation day 3 E2 33.16±1.51 pg/ml, FSH 10.90±0.96 mIU/ml, OV day E2 254.42±18.1 pg/ml, LH 68±2.07 mIU/ml, P4 0.68±0.07 ng/ml, WOI day E2 25.86±3.12 pg/ml, P4 16.52±0.96 ng/ml, endometrial thickness 10.47±0.23 mm, menstrual cycle 30.19±0.23 days, respectively, were not statistically significant. However, according to the histomorphological method of Noyes et al. (1950), 10.4% (n=5) 3, 333% (n=16) 2, 33.3% (n=16) 1 days delay, 16.7% (n=8) expected day statistically significant activity. Comparison of histological maturation with clinical days after ovulation showed a delay of endometrial maturation varied individually, occurring 1-3 days late. Conclusion: endometrial maturation requires 8 days, rather than the expected 6 days, to reach the histological mid-secretory phase. The new analysis method used is superior to that using Noyes criteria alone and provides a better basis determining conditions for optimal timing of embryo transfers.
