Diagnostic issues of acute appendicitis in adult
Keywords:
Appendix, Acute inflammation, Alvarado score, RIPASA scoreAbstract
Background: As of 2020, there were 12,242 cases of acute appendicitis (AA) in Mongolia. AA is the most common type of abdominal surgery that requires emergency care. The aim of our study was to compare the pathological changes of the surgically removed appendix with some diagnostic scoring system and the time of pain. Materials and Methods: This study was performed using an instantaneous survey model. The study collected clinical data from the medical history of patients who underwent surgery for AA in the surgical department of the General Hospital for State Special Servants from October 1, 2020 to June 1, 2021, and evaluated appendicitis microscopy. Results: We compared the 99 cases in our study with pathological diagnoses, 84.8% (n=84) did not have acute appendicitis and 15.2% (n=15) did not show any signs of acute appendicitis. The total number of cases of gangrenous inflammation increased with time from the onset of pathologically confirmed cases of abdominal pain and in the presence of surgically amputated forms of appendicitis, and there was a statistically significant difference between the type and duration of inflammation (p=0.049). Conclusion: The cut-off value of the RIPASA scoring system of the respondents was 7.5. The cut-off value of the Alvarado scoring system was 7 points, which increased the sensitivity and specificity of the diagnosis. Estimating the onset of abdominal pain in patients is important in predicting the type of appendicitis.
