Introduction: In Mongolia, ischemic heart disease-myocardial infarction remains the leading cause of mortality. N-terminal pro-brain natriuretic peptide (NT-proBNP) is released from cardiomyocytes due to wall stress and is a main biomarker of heart failure in guidelines. Purpose: Aim of this study was to observe NT-proBNP release in acute myocardial infarction (AMI) and investigate factors that affecting its serum concentration. Materials and Methods: We enrolled 120 patients with AMI without clinical heart failure with median NT-proBNP level of 838 (IQR 280-2860) pg/ml. Results: According to NT-proBNP quartile patients divide into 4 groups and there were significant elevation of troponin in higher quartile group (p< 0.001), as well as lower global longitudinal strain GLS (p<0.045). The serum NT-proBNP values were associated with diabetes mellitus OR 4.017 (p=0.025), shortened hospital admission OR 0.05 (p=0.02), higher GRACE score OR 1.053 (p=0.038) and increase of GLS OR 0.693 (p=0.008). Conclusion: NT-proBNP concentration in AMI patients is related to early left ventricular systolic dysfunction. Thus beside echocardiographic examination, elevation of NT-proBNP could be used for prompt left ventricular systolic function.
