Association between allergen sensitization and detection of lower airways obstruction by mannitol challenge test

Background: Prevalence and morbidity of asthma is dramatically increasing in the last decades and there are 339 million people with asthma globally. In our country, asthma morbidity and prevalence is increasing year by year. In 2010, prevalence of asthma among 6-7 years old children is 20.9% and current asthma is 15.8%, physician diagnosed asthma is 4.7% among adults in our country2-6. We have a possibility to detect asthma earlier with airways hyperresponsiveness by the bronchial challenge test. Aim: The main aim this study was to defined association between aeroallergen sensitization and detection of airways hyperresponsiveness by mannitol challenge test. Method and materials: We were detected 40 different allergen-scpecific IgE by the automated microfluidic-based immunoassay system (Agnitio Technology and Sciences Inc, Taiwan) and for detection of airway ventilation function by the spirometry (Spirostar USB, Spiro pro 3.1.2 software Medikro OY). Mannitol challenge tests were used 9 steps protocol (Aridol/Osmohale, Pharmaxis Ltd Frenchs Forest, Sydney Australia) and cumulative doses was increased 635 mg. We were evaluated the significant decreasing of FEV1 was 15%. Results: We were chosen 196 subjects with 18-69 years old (mean age 32±8.8, 95%CI: 23.4–49.6) 95 males and 101 females. Allergic asthma subjects were sensitized to pollen 163 (83.1%), animal dander 24 (21.6%), latex 24 (21.6%), dust mites 8 (9.6%). Allergen specific IgE of peripheral blood were detected strongly sensitized to weeds 9.2±6.7 AU, medium sensitized to horse (6.5±4.2AU), cat (5.9±4.5AU), grasses (5,75±3.5 AU), wheat (5.6±4.4 AU) and birch (5.5±4.0 AU). There was statistically significant decreased of postFEV1 than preFEV1 parameters in male 48.3±8.6% and in female 38.3±9.6% by the mannitol challenge test. Conclusion: The season of mugwort pollination period of time was associated with bronchial hyperresponsiveness detected by mannitol challenge test.
