Abstract

Background: Radicular cysts are the most prevalent kind of odontogenic cyst found in periapical lesions, accounting for 6% to 55% of identified cases. Radicular cysts are treated in two stages: decreasing the cyst’s internal pressure followed by cystectomy. By decreasing the cyst’s internal pressure and size that reduces the probability of fundamental anatomical structure damage, and the cyst’s outer membrane thickens which enhances outcomes of second surgery. Materials and Methods: Two participants, (17-year-old female with swelling in the upper incisor gum and a 33-year-old female with substantial swelling in the left palate region) visited MNUMS’ Central Dental Hospital. In both cases, CBCT was used to detect the root canal and to supply calcium hydroxide for six months. The apicoectomy was performed under regional anesthesia; the root apex has been removed 3mm and retrogradely prepared with an ultrasonic E10D tip and it was filled with calcium hydroxide-containing silicate-based cement biomaterial using endomicroscopy. Results: The use of calcium hydroxide in root canal treatment reduced cyst size and surrounding bone regeneration by 20%-23% in both cases. Conclusion: Endodontic treatment using calcium hydroxide in the first stage of treatment has a beneficial effect by reducing the size of cyst, internal pressure of cyst, thus increasing the overall success.
