Background: Early detection of COVID-19 disease and proper evaluation and analysis of clinical manifestations are important to reduce complications and mortality caused by SARS-CoV-2 virus. At the beginning of the COVID-19 pandemic, Mongolia did not spread domestic infections for 8 months by screening, isolating, and treating everyone who came from abroad. Aim: To compare the clinical presentation of domestic and transported cases of COVID-19. Materials and Methods: 276 cases of COVID-19 transmitted from abroad and 1024 cases of domestically spread infection were included in the study. In the research material, the medical history of patients hospitalized in the National Hospital of the National Medical Center was used, and the results were analyzed with the R Studio 4.1.3 program. Approval was obtained at the 2021/3-06 meeting of the Ethics Control Committee of MNUMS. Results: In transmitted cases of COVID-19, younger age (mean age 26.5±11.2) and asymptomatic form (36.9%) occurred more often than in domestic infections (mean age 38.9±21.7 years, p<0.001 and 12.1%, p<0.001). In terms of gender, males predominated (67.8%) in transmitted cases, while females predominated (57.8%) in domestic infections (p<0.001). Symptomatic form was 63.1% in transmitted infection, 87.9% among domestic cases, mild clinical form was 30.5% and 28.5%, moderate - 32.6% and 37.8%, while severe and very severe form was not recorded in transmitted infection, 18.5% in domestic COVID-19 disease. and 3.1% respectively. Clinically, cough symptoms were found in 19.2% cases, while in domestic infections - 60.9%, fever -13.4% and 29.2%, headache -14.9% and 33.7%, nausea - 3.9% and 12.2%, chest pain - 5.1%. and 20.8%, shortness of breath -1.1% and 27.3%, or clinical symptoms were more pronounced (p<0.001) during domestic infection. Conclusion: As a result of the early initiation of measures to prevent the spread of the COVID-19 epidemic in Mongolia, asymptomatic and mild forms prevail among transported cases.
